Responsibilities
- Conduct psychosocial assessments to determine patients' emotional, social, and financial needs.
- Develop and implement care plans to address patient concerns.
- Provide counseling and crisis intervention for patients and families.
- Advocate for patient rights and assist with healthcare decision-making.
- Coordinate discharge planning and transition of care services.
- Educate patients and families about community resources and support programs .
- Maintain detailed case records and ensure compliance with healthcare regulations.
- Collaborate with healthcare providers to ensure holistic patient care.
Qualifications
- Education: Master's degree in Social Work (MSW) .
- Licensure/Certification:
- Licensed Clinical Social Worker (LCSW) or equivalent state licensure.
- Additional certifications in healthcare social work preferred.
- Experience:
- 1+ years of experience in medical or healthcare social work.
- Skills:
- Strong advocacy and problem-solving abilities .
- Excellent communication and counseling skills .
- Knowledge of healthcare systems and patient rights .
- Travel Readiness: Willingness to relocate temporarily for assignments.
